As an Artist Member, I agree to paying a 20% commission on all sales made to the BAFFA Art Gallery. I understand that BAFFA will collect all sales upfront and at the close of my exhibition, BAFFA will submit the proceeds (minus the 20% commission) to me within 30 days by mail.

I agree to release Bay Area Friends of the Fine Arts of any liability that may be incurred during the phase of the exhibition of the above-listed artworks at the BAFFA Art Gallery during the period of my exhibition. I take full responsibility for any damage or loss from any cause that may occur to any of my artwork being exhibited during the period of my exhibition. I understand that I must insure my artwork if liability coverage is desired. I approve that any photos or video of my artwork(s) or myself will be used for promotional purposes, including but not limited to Instagram, Facebook, BAFFA website, newspapers, etc.

I agree to pay the Artist Fee toward the creation and printing of my posters. If you are filling this form out for an Artist Group (another NFP, association, etc), and paying for your own posters, then you agree to provide your own posters after approval from the Gallery Director. Please check the appropriate field below, if you are part of a one, two, or three artist exhibition. (Only ONE fee of $25.00 needs to be paid PER exhibition, not per artist exhibiting over the month.)

Please choose the appropriate payment based on how many artists are exhibting:

One Artist Payment - $25

Two Artist Payment - $12.50

Three Artist Payment - $8.50
